A Clinico-Pathologic Characteristics and Prognosis in Patients with Malignant Ovarian Tumors

Abstract
This study was performed to evaluate the clinico-pathologic characteristics and prognosis in patients with malignant ovarian tumors. Two hundred fifty-five patients with malignant ovarian tumor, who had been treated at the Korea Cancer Center
Hospital
from Jan. 1987 to Apr. 1995, were studied retrospectively. The results were obtained as follows. According to histo-pathologic classification, malignant epithelial tumors were found in 193(75.7%) out of 255 cases ; germ cell tumors, in 31(12.1%)
cases ;
and sex cord tumors, in 15(5.9%) cases. Patients ranged in age from 9 to 73 ; the mean age was 45.1 years. In relation to FIGO stage, 73(50.0%) of 193 patients with malignant epithelial ovarian tumor were classified to stage III ; 55(37.6%) to
stage I ;
12(8.2%) to stage II ; and 5(3.4%) to stage IV. Among malignant epithelial ovarian tumors, serous cystadenocarcinma was the most frequent(49.3%) distinctive histologic type, followed by mucinous adenocarcinoma(29.4%), endometrioid
carcinoma(2.1%).
In
the germ cell tumors, dysgerminoma comprised the highest proportion(41.9%). In univariate analysis, age, stage, and residual mass were found statistically significant prognostic factors in survival ; age(<50 vs>49,79 vs 65%, p=0.0293) ; stage(I
vs
II
III/IV, 90% vs 78% vs 57%, p=0.0005) ; residual mass(none vs <2cm, 89% vs 70% vs 53% p=0.0005). In multivariate analysis, only residual mass(p=0.0208) was found to be an independent significant prognostic factor in survival of patients with
malignant
epithelial ovarian tumors. These results show that epithelial histologic type comprises about one-third of malignant ovarian tumors, and residual mass size after primary surgery was the most important prognostic factor of survival histologic
entity.
